Understanding Post-Stroke Fatigue

Fatigue after stroke is one of the most common and least understood things people experience. Somewhere between 4 and 7 out of every 10 stroke survivors report real fatigue in the months afterward — and for many, it sticks around for years. Unlike ordinary tiredness, it doesn't reliably go away with rest, and it doesn't match how much you've actually done. It can arrive suddenly, at unpredictable times, and people often describe it as feeling different from any tiredness they've felt before.

We don't fully understand why it happens. Current thinking points to a few things that likely contribute: the extra work your brain is doing as it reroutes around the damage; disrupted sleep (stroke commonly affects how deeply and how well you sleep); the physical cost of your body repairing and healing itself; and, for some people, low mood, which can make fatigue feel worse.

What is clear is that fatigue after stroke is not a lack of motivation, a character flaw, or a sign that you're not recovering well. It's something happening in your brain and body that responds to the right management — including, importantly, deliberate rest at the right moments.

The 90-Minute Rest Cycle

Research shows the brain naturally shifts between higher and lower alertness roughly every 90 minutes, all day long. You can see this pattern clearly in sleep, where it shows up as different sleep stages — but it keeps going during the day too, as small shifts in how alert, focused, and open to rest you are.

In a healthy system, short periods of lower activity at each low point let the brain process what it's taken in and get ready for the next active stretch. After stroke, this natural rhythm is often disrupted, and pushing through those low points tends to cost you more than usual. This practice is built to support real, good-quality rest at those natural low points — instead of pushing through them, which tends to backfire.

You may notice your fatigue arrives in waves. If you can work out roughly when your low points happen (commonly mid-morning and mid-afternoon), those are good times for this practice.

How This Practice Differs From the Others

The other four practices on this site use guided imagery to rehearse movements — reaching, walking, balancing, speaking. This practice does something different. It doesn't ask you to rehearse a movement. It asks your nervous system to do something that fatigue after stroke often gets in the way of: to properly rest.

The research behind this practice comes mainly from relaxation and mind-body studies, not from motor imagery research like the other four. Guided relaxation for fatigue after stroke has shown positive results in several studies — including improvements in how fatigued people feel, their mood, and their sleep — though the improvements are modest and the studies vary in quality. The approach used here (extended guided relaxation with body-based imagery) is backed by research into the body's rest-and-recovery systems and the restorative value of deliberate rest.

Part III of this practice doesn't trace a path to a limb, the way the other practices do. Instead, it draws your attention to the deep brain systems that control alertness and rest, and to the slow, restorative processes of genuine rest. The imagery stays gentle and calming throughout, rather than energising.

Part I · Breath and Letting Go
Permission to stop

What it's for. Fatigue often comes with a background alertness that makes it hard to settle, so this opening is longer and slower than the others.

How it works. You lie down if you can, let the surface take your weight, and lengthen each out-breath until your body begins to accept that it's safe to let go.

Part II · The Ideal Being
Someone who knows how to rest

What it's for. For this practice, the Ideal Being isn't moving — they're resting, deliberately and without guilt.

How it works. You bring to mind yourself at rest — breathing easily, body warm and heavy, face unworried — and let your body begin to move toward that state.

Part III · The Restorative System
The brain at work while the body rests

What it's for. This practice doesn't rehearse a movement pathway — it rehearses your brain's own systems for winding down and clearing out the day.

How it works. Your attention goes to the systems deep in your brain that control alertness, and follows them slowing — like a tide going out. Falling asleep here is a valid outcome, not a failure.

This practice has no route from nerve to muscle to follow, so this is a picture of an idea rather than a map of anatomy. Settling is gradual and partial rather than a switch, and falling asleep during the practice is a valid outcome, not a failure.

Part IV · Closing
Coming back slowly

How it works. Your breath deepens slightly, movement starts very small, and your eyes open softly. You take three slow breaths and get your bearings before standing.

This practice is particularly well suited to audio — reading it yourself while fatigued is counterproductive. The written script below is included for reference, and for reading aloud by a family member or carer.

A Note on the Audio

The audio narration is recorded by me, in my own voice. The words and pacing are mine, and no synthesised or computer-generated voice is used. If listening doesn't suit you, the written script below is equally effective — read it quietly to yourself, or have someone read it to you. Many people find their own inner voice the most effective narrator.

The Script

This script is written to be read aloud to you, or listened to via audio narration. If you need to read it yourself, read it very slowly — more slowly than feels normal. Allow long pauses between paragraphs. The quality of attention this practice requires is soft rather than focused.

Part I · Breath and Letting Go

Find a place to rest. Lying down is best for this practice — in bed, on a couch, or on the floor if that is comfortable. If lying down is not possible, a supported chair is fine. Close the curtains if the light is bright. Loosen anything that presses or constricts.

Let the weight of your body settle. You do not need to hold anything up. The surface beneath you will do that.

Allow your eyes to close.

For a moment, just notice. The sound of your own breathing. The warmth of the room. The surface you are resting on against your back, or your side. The small sounds in and around the building. None of these require a response. They are simply the texture of the moment you are in.

Now bring attention to the breath. Not to change it — just to notice it is happening. The small rise and fall. The slight pause. The release. The body already knows how to breathe. It has been breathing this whole time without your attention, and it will continue to. You are simply arriving alongside it.

Begin to lengthen the breath out. Not by forcing it — simply by letting the exhale take a little longer than before. As if sighing, but quietly. Let the out-breath become a release.

With each out-breath, something softens. The muscles around the eyes. The jaw — let it drop slightly, releasing whatever it has been holding. The neck and shoulders. The hands — uncurled, open, resting.

There is nothing you need to do right now. No task to complete, no performance to give. The only thing this time asks of you is to be here, and to rest. You are allowed to rest. The brain cannot repair and recover if you do not allow it to rest. This is not inactivity. This is the work.

Continue with slow, easy breathing for a few moments. Let each out-breath be a little fuller. Notice the body growing heavier and warmer with each breath. This is the correct direction.

Part II · The Ideal Being

Bring to mind yourself — simply yourself — resting. You may see it, or simply sense it; either is right. Not ill, not recovering, not struggling. Just resting, as if rest were the most natural thing in the world. Because it is.

You are lying or sitting somewhere comfortable and familiar. The breath, slow. The body, warm and heavy. The face, smooth — nothing tightening around the eyes, no clench in the jaw. The hands, open and still.

This is not inactivity. This is a version of you engaged in something important: allowing the nervous system to do what it does when it is not being demanded of. Consolidating the learning of the day. Clearing the by-products of sustained effort. Restoring the capacity to re-engage.

Notice what it is like in the body to rest like this. Perhaps a small settling — a slight deepening of the breath, a release in the belly or the chest. Allow it. The body recognises rest, even in imagination, and begins to move toward it.

If the sense of it is vague — if you cannot easily find yourself at rest — that is fine. Simply rest in what is here. The warmth. The weight. The slowing. The permission.

Part III · The Restorative System

Now, very gently, bring attention to the inside of your head. Not to any particular thought. To the sense of the brain — warm, complex, working, even now.

Deep in the centre of the brain, in the brainstem — the oldest, deepest part of the nervous system — there are structures whose job is to manage the shift between waking and rest. A network threaded through the brainstem like a net, setting the level of alertness of the whole brain above it.

That system is beginning to slow. Not stopping. Slowing. Like a river moving from open water into still, deep pools. The signals that have been firing all day — the alertness, the monitoring, the effort — beginning, very gradually, to ease their pace.

The brain does not stop working when you rest. It shifts into a different kind of work. Its waste-clearance system becomes more active in rest and sleep than in waking. The channels between the cells widen slightly, and the by-products of a day's effort are gently flushed through and carried away. The brain is cleaning itself. This is what rest is for.

That process, now. Warm, slow, fluid. The space between cells widening, just slightly. A gentle, internal tide of clearing and renewal — so quiet as to be nearly imperceptible, and utterly fundamental.

The things you learned today are settling into place — what you practised is being stored, organised, made easier to access. The pathways you rehearsed in your other practices today are being reinforced now, in rest. Rest isn't the opposite of learning. It's the completion of it.

Let the breath continue — slow, involuntary, complete. Let the body be heavy. Let the mind wander if it wishes, without following it. Let thoughts pass like cloud shadows on still water — present, then moving on, without disturbing the surface.

There is no requirement to stay awake. If sleep comes, allow it. If the mind stays gently present, allow that too. Both are rest. Both are correct.

Stay here as long as the practice allows — five minutes, ten, whatever your circumstances permit. The longer and more complete this rest, the more restorative it is. Twenty minutes of deliberate, guided rest is a different thing from twenty minutes of lying down while worrying. The difference is in the state of the nervous system — and that state is what you have been cultivating throughout.

Part IV · Closing

When you are ready to return — and there is no urgency — let the breath deepen slightly. Not forcing a return. Simply becoming a little more present to the body and the room.

Bring awareness gently to the surface you are resting on. The temperature of the air. Any sounds that are present.

Begin to move, very slowly — a small movement of the fingers or toes. A gentle shift of weight. No need to move quickly. The transition from rest to activity, made slowly, is itself part of the practice.

When you are ready, let the eyes open — softly, without snapping back to alertness. Let the light come in gradually.

Before you stand or take on any task, take three slow breaths and allow a moment of orientation. Notice how you feel. You may feel groggy and heavy, which is normal and will pass. You may feel lighter than before. Both are valid. What matters is that the rest window happened — that the nervous system was given the opportunity it needed.

Your body moves through natural rhythms all day — roughly every 90 minutes, attention naturally softens. Those moments are good ones to return, briefly, to this sense of permitted rest. And the transition points — falling asleep tonight, first waking tomorrow — are when this kind of rest consolidates most easily. A few breaths of it there carries the practice further than the same breaths at midday.

That is the practice. Take it at the pace your body asks for. There is no performance here.

This practice sits alongside your rehabilitation team's guidance on rest and energy — it doesn't replace it. It's a rehearsal, offered to your body as it is today.

When to Use This Practice

Primary use
At the fatigue trough

The best time is when you first notice fatigue starting — not once you're already worn out. For most people, natural low points happen mid-morning (around 90–120 minutes after waking) and mid-afternoon. Rest before you're exhausted, not after — that's the idea.

Secondary use
After demanding practice

After any of the other four guided imagery exercises, or any sustained physical or mental effort, a shorter version of the opening and Part III of this practice helps things sink in. Even ten minutes of deliberate winding-down after active practice improves how well you retain what you've learned.

Third option
Before sleep

This practice can also work as a wind-down before sleep. Disrupted sleep is common after stroke. Deliberately calming your nervous system before bed — instead of passive screen time — can help you fall asleep faster and improve the quality of your early sleep. Falling asleep during the practice is welcome.

A Note on Guilt and Rest

Many people after stroke describe feeling guilty about resting — as if resting meant giving in to the injury, or slowing recovery down. That's understandable, but it's actually backwards. Recovering from a brain injury depends on rest just as much as on active rehab. The changes in the brain that make recovery possible — new connections forming, other brain areas taking over lost jobs — take a lot of energy, and that work happens largely during rest and sleep.

If you have family members or carers who find it difficult to see you resting, sharing this page with them may be useful. Resting after stroke, deliberately and regularly, is not passive. It is an active component of recovery.

An Honest Note About This Practice

This practice is different from the other four in what evidence it's based on. The mental-practice research behind the arm, walking, balance, and speech practices isn't the main source here. For fatigue specifically, the evidence points toward relaxation, managing rest, and psychological approaches (including mindfulness and cognitive behavioural techniques) rather than mental practice of movement.

This practice draws on research into guided relaxation for fatigue after stroke (which is positive but modest), the science of 90-minute rest cycles, and research into how the brain clears waste during rest. The brain-focused imagery in Part III is adapted from the four-part structure used across this whole programme — grounded in real science, but not itself tested in a clinical trial. This practice differs from the other four: its Part III follows the brain systems that control rest rather than a path to a limb, and its Ideal Being is resting rather than moving.

If your fatigue is severe or is seriously affecting your quality of life, it's worth talking to your medical team. Treatable causes — including depression, sleep problems, low iron, and thyroid issues — should be ruled out before assuming the fatigue is simply a result of the stroke itself.
