What This Practice Helps With

Balance problems after stroke are common and can have real consequences. Falls are one of the leading causes of injury after stroke. Less obviously, feeling unsteady on your feet also makes people do less — less exercise, less socialising, less of anything that means standing or moving without help.

Balance isn't a simple skill. It relies on three different senses working together — your eyes, the balance organs in your inner ear, and the sense of where your joints and muscles are — all coordinated by a part of your brain called the cerebellum. A stroke can disrupt any part of this system. Imagery for balance works a bit differently from imagery for arm or leg movement: instead of tracing one simple pathway to one muscle, this practice asks you to rehearse the overall feeling of being centred and steady, rather than one specific sequence of muscles.

There's less research specifically on imagery for balance than for the arm or walking, but what exists is positive. Studies that combine mental practice with balance training find bigger improvements than balance training on its own. The likely reason is the same as for arm or leg imagery — rehearsing the brain connections that the skill depends on.

Part I · Breath and Light
Settling into the Body

What it's for. Balance depends on knowing where your weight is, so this practice builds that awareness first.

How it works. You let the surface beneath you take your weight entirely, slow your breathing, and notice where your body makes contact. Warmth travels from the top of your head down to the base of your spine — your centre.

Part II · The Ideal Being
Watching Someone Stand

What it's for. For this practice, that means watching a stance that isn't guarded or braced — then feeling that steadiness as your own.

How it works. You watch a figure simply standing: weight even, spine long, unhurried. They sit and rise again, easily. The figure then steps into you, and the steadiness becomes yours.

Part III · The Anatomical Pathway
The Balance Network

What it's for. For this practice, the pathway isn't one route to one muscle — it's the whole balance system working together.

How it works. Your attention goes to the cerebellum at the base of your skull, then to the signals reaching it — from your inner ear, and from your ankles, knees, and hips — and then out again to the muscles that hold you upright.

The network this practice follows: signals from the eyes, the inner ear and the joints of the leg converge on the cerebellum, and corrections travel back out to the muscles that hold you upright.

A simplified schematic rather than an anatomical reference, and a network rather than a single route. Only one side is drawn. Balance is continuous and two-way: the body reports where it is and the brain corrects, many times a second, without your attention.

Part IV · Closing
Resting in Centre

How it works. You return to your breath and to wherever you actually are — sitting, lying, resting — and take a few long breaths before finishing.

Part I · Breath and Light

Settle into whatever is supporting you right now. Let the surface take your weight. There is nothing to hold up in this practice. Let it all down.

Take a slow breath in. Let it out slowly. On each out-breath, let a little more of the body's held tension release. The shoulders. The jaw. The hands.

Bring warmth to the top of your head. Let it descend with each breath. This time, let the warmth travel all the way down to the base of your spine — to the pelvis, the sitting bones, the very foundation of your body. The weight of you is here. The centre is here.

Part II · The Ideal Being

In front of you there is a figure, standing. You may see them, or simply sense them there — either is right. They are simply standing: not performing, not bracing. Their weight is even. Their spine is easy and long. There is a soft, relaxed steadiness about them.

Notice what this is like when it is not effortful. The slight, continuous movement of the body — barely there — as it makes small adjustments to stay upright. The trusting of the ankles. The quiet hips. The calm in the shoulders. No gripping. No compensating. Just a body that knows where it is in space.

The figure steps into you. Now you are standing — or imagining standing. Either is fine. The ground under your feet. The weight distributing. The length in the spine. The head sitting easily at the top of it all. You are upright. Centred. The balance is happening. It doesn't need managing.

Part III · The Anatomical Pathway

Bring your awareness to the back of your skull — the base of your brain. Your cerebellum is here. It's always working, always receiving signals from your inner ear, your joints, your muscles. It's the coordinator of balance. Sense it there — not a sharp picture, just a sense of something quietly, steadily active.

Bring attention to your inner ear. The fluid inside it, reporting the position of your head. The parts that sense gravity. This information is flowing to your cerebellum right now. Let it flow. Don't interfere. Just notice it.

Now bring attention to your joints — wherever they are, whatever position they're in. Your ankles. Your knees. Your hips. Each one is sending signals upward, reporting its angle, its tension, its load. Your cerebellum takes it all in. Puts it together. Sends correcting signals back out to the muscles that hold you upright — the deep muscles along your spine, the muscles around your hips, the small muscles in your ankles.

Now sense yourself upright. Standing. The whole system working the way it was built to. Signal in, signal out, balance kept — not because you're gripping or thinking hard, but because the system is running. Let it run.

Part IV · Closing

Return to the breath. Return to wherever you are — sitting, lying, resting. You are supported. The practice has been received. The imagery settles.

Carry a fragment of this today: the spine long, the weight dropping through the feet, the balance just happening. In any window of quiet — a slow breath, a pause — return to it.

Your body moves through natural rhythms all day. About every 90 minutes, your mind drifts — when you notice that drift, return for a breath or two to the feeling of standing, centred, easy. At night, as you drift toward sleep, the practice is especially potent. A few breaths of that easy balance as you fall asleep carries the work into your dreams. The same is true when you first wake.

Three long breaths now. The room around you. Enough.

This practice sits alongside your physiotherapy — it doesn't replace it. It's a rehearsal, offered to your body as it is today.

When to Practise

Throughout the day
Before transitions

Before you stand from sitting, spend one breath imagining the movement — the weight shifting forward, the legs activating, the rise. This preparation imagery may improve motor readiness and reduce hesitation.

During rest
The 90-minute drift

Return briefly to the felt sense of being centred and upright. Two or three breaths of the spine long, the weight even, the balance easy. The image accumulates with repetition.

Before sleep
The standing image

Hold the image of standing — centred, easy, unguarded — as you fall asleep. The cerebellum integrates motor information during sleep. This practice seeds that consolidation.

On waking
Before you first stand

The minutes right after waking are a natural pause before the day's first standing and walking. Before you get up, spend a few breaths on the image of standing centred and easy. It's a moment already built into your morning, which makes it easy to remember.

An Honest Note

The evidence for mental practice specifically helping balance after stroke is positive, but there's less research on it than there is for the arm and hand. Studies have found improvements in balance measures when imagery is added to balance training. The specific cerebellum-focused imagery used in this practice is a reasoned approach, not a tested method.
